Udoka began his professional basketball career with the Fargo-Moorhead Beez from 2000 to 2001 after being left off of the 2000 NBA draft.

Udoka was selected with the 39th overall choice in the 2002 NBDL Draft by the Charleston Lowgators, whom he played for in the league. On January 14, 2004, he received a call-up to play with the Los Angeles Lakers, but he was eventually cut.

Udoka committed to a deal with the San Antonio Spurs in 2007. In 73 games, he averaged 5.8 points, 3.1 rebounds, and 18 minutes of play. Udoka participated in 67 games during his second season with the Spurs, starting three of them. In 15.4 minutes, he averaged 4.3 points and 2.8 rebounds.

Udoka became a free agency after the 2009 season and ultimately re-signed with the Trail Blazers. On October 22, 2009, the Blazers waived him.

On November 4, 2009, Udoka signed a contract with the Sacramento Kings. During his 69 games with the Kings, he averaged 13.7 minutes, 3.6 points, and 2.8 rebounds.

He returned to the Spurs on November 24, 2010, however after just 20 games, on January 5, 2011, he was dismissed. Udoka signed a contract with the New Jersey Nets on December 15, 2011.

Nevertheless, on December 23, 2011, he was released. Udoka signed a contract with UCAM Murcia of the Spanish Liga ACB in January 2012.

Udoka joined the San Antonio Spurs in August 2012 to serve as Gregg Popovich’s assistant coach. In the 2014 NBA Finals, the Spurs defeated the Miami Heat 4-1 to give Udoka his first championship.

LaMarcus Aldridge’s 2015 choice to join the Spurs was also influenced by Udoka. During Aldridge’s debut campaign with the Trail Blazers, Udoka and Aldridge were teammates.

Udoka joined the Philadelphia 76ers as an assistant coach on June 26, 2019. In addition to having worked under Gregg Popovich with the Spurs, Brett Brown, Udoka’s head coach with the 76ers, was also Udoka’s assistant coach during the 2012–13 campaign.

Udoka became the fifth head coach of African descent in the history of the Boston Celtics when he was appointed on June 28, 2021.

Despite the Celtics’ 18–21 start to the season, Udoka guided the group to a 51–31 record and the second seed in the Eastern Conference. The Celtics advanced to the NBA Finals for the first time since 2010 after winning the Eastern Conference. But the Golden State Warriors defeated the Celtics in six games.

The Houston Rockets appointed Udoka as head coach on April 25, 2023. Udoka was sent off for trash-talking Los Angeles Lakers forward LeBron James on December 2 during a 97–107 loss at Arena.
